Responsibilities The Focus program manager is responsible for the coordination planning and implementation of California Hospital Medical Center Focus program. The program will increase screening for HIV Hepatitis C and Hepatitis B for patients in the Emergency Department and will provide linkage to care for those with positive results.
- Work with colleagues across the organization to design plan and implement FOCUS Program
- Establish policies and procedures and implement best practices for the FOCUS Program
- Hire train and supervise the Care Patient Services Navigator
- Must be able to present program data and outcomes in various settings
- Coordinate logistics materials and scheduling for staff training
- Coordinate facilitate and follow up on internal FOCUS Task Force meetings
- Assure program compliance with stated work plans and objectives expectations budgets and reporting requirements for grants and contracts; prepare reports and budget modifications for funds as requested
- Research best practices and changes in the field of HIV HCV and HBV screening and treatment
- Create and maintain standardized and appropriate education and outreach materials
- Foster and maintain relationships with community partners
- Attend local and regional meetings and events related to FOCUS
- Develops process to disclose positive diagnosis to patients ensuring that patients are linked or re-linked to the clinic as necessary and provides health education and HIV prevention information
- Presents program and outcomes in a variety of settings
Qualifications - Bachelor's degree OR minimum 2 years of experience in HIV-related field
- 2 years of management/ leadership experience
- LA Fire Card (within 90 days of employment).
- Strong communication skills both oral and written
- Able to work independently with minimal supervision
Preferred
- Bachelor's Degree in social services health education or related field
- Experience and cultural competency with high-risk groups and providing services to high risk individuals homeless or underserved populations
- Knowledge of community resources and referral agencies
