DescriptionGENERAL DESCRIPTION: Performs daily duties of resident care, per job standards, as directed by team leader in accordance with prescribed procedures and policies of the facility and Federal, State and local authorities, to ensure that quality nursing services are provided at all times. MAJOR DUTIES AND RESPONSIBILITIES: 1. Maintain the standard of nursing care and follow the policies and procedures of the facility. 2. Protects and maintains confidentiality of residents, information, and residents rights. 3. Respects and shows kindness to all residents. Supports person centered care concepts. 4. Attends team care conferences as needed. 5. Reports all pertinent information to team leader and appropriate support staff. 6. Records all pertinent information in residents record. Examples being: EMR's, Restorative Care, Food Intake records, behavior notes and other written documents. 7. Protects and maintains all work areas in accordance with facility safety policies. 8. Assures that floors, resident rooms, common areas, and service rooms are free of hazardous debris. 9. Performs daily tasks in accordance with infection control and standard precaution policies and procedures. 10. Makes beds daily per facility policy. 11. Answers call lights and alarm sounds promptly. 12. Follows policies and procedures related to body mechanics and lift system. 13. Follows policies and procedures related to positioning, transfers, and mobility. 14. Ambulates residents in accordance with individual plans of care. 15. Prepares, assists and transports residents to and from meals, activities and therapy sessions. 16. Assists, supervises and feeds residents at meal time. 17. Bathes residents in accordance with plans of care. 18. Assists residents with dressing and undressing. 19. Combs hair, shaves and applies appropriate appliances for residents. 20. Gives good oral care and observes for oral problems to report to nurse. 21. Performs and assists with passive, active ROM exercises, and Restorative Programs 22. Gives lotion back rubs. 23. Passes fresh ice and water, extra fluids and AM or HS nourishments. 24. Takes and records weight, temperatures, fluid intake and output, vital signs and other pertinent data. 25. Bathes residents as assigned. 26. Assures that residents have necessary personal care items. 27. Checks that bed pans, urinals, and collection hats are stored according to facility policies. 28. Assures that BSC, bed pans, urinals, and hats are cleansed and dried, per facility policies. 29. Toilets residents and/or conducts proper peri care per resident's plan of care/policy. 30. Dresses in accordance with facility dress code policy. 31. Completes walking rounds with shift leaving, when reporting to work and shift coming on when leaving work. Observes each resident assuming care for to ensure safety and continuity of care. 32. Participates in facility committees giving input including QA/PI. 33. Performs all other duties according to STNA certification. 34. Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator. 35. Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator. 36. Report any known or suspected unauthorized attempt to access facility's information system. ANCILLARY RESPONSIBILITIES: 1. Assures that residents' belongings are in appropriate place. 2. Straightens closets, places residents clothing on hangers. Straightens and arranges drawers appropriately. 3. Assists in taking vital signs and blood sugar levels for RN/LPN as needed. 4. Restocks and maintains order of linen cart, service rooms and common areas. 5. Assists with transport of residents to and from doctor's appointments as directed by supervisor. 6. Is flexible to changes in schedule and works overtime as needed. ASSIGNMENT, REVIEW, AND APPROVAL OF WORK:1. Receives oral instructions from team leader. 2. Shows initiative and judgment in doing assigned tasks. 3. Able to perform, organize, and evaluate work. 4. Work may be reviewed by team leader/DON/ADON/RN, during or upon completion of shift. RESPONSIBILITY AND DECISION-MAKING AUTHORITY: Decision making is limited to specific responsibilities as assigned by team leader. REPORTING PREPARATION: 1. Oral and written report as prescribed in policies. 2. All changes in residents' condition. 3. All hazardous conditions in the facility, including equipment. 4. All accidents or incidents observed. EDUCATION & EXPERIENCE: 1. Must be registered as a State Tested Nursing Assistant. 2. Work permit required if under 18 years of age. 3. Maintains STNA certification by achieving 12 hours of in-service credit per year based on hire date. 4. Regularly attends in-services, nursing/staff and QA/PI meetings. EQUIPMENT OPERATION: Lifts Recliners Glucometers Wheelchairs Geri-chairs SDC's CPM machines Oxygen concentrators Oxygen tanks/Regulators Air Mattresses SPECIFICATIONS - MENTAL/PHYSICAL: 1. Work/tasks are performed mostly indoors. 2. Escorts residents outdoors for on/off ground activities. 3. Outdoor activity areas are well-lighted and clean, with some exposure to dust, dirt, and organic substances. 4. Heat, air conditioning and humidity are controlled by the facilities central system with variations in temperature on either side of the normal 72 degrees.
  • There are constant exposures to blood, body tissues and fluids, with occasional
  • exposure to hazardous materials and infectious diseases.
  • Adjusts to exposure of loud and unpleasant voices.
  • Exposure to constant stress of working with sick residents and their families and
  • regulatory compliance teams.
  • Exposure to residents who are confused, irrational, agitated, and combative with
  • abrupt mood swings on a regular basis.
  • Lifts, routinely objects weighing up to 100 pounds.
  • Stands and walks on hard surface 6-7 hours daily.
  • Bends, stoops and turns often.
  • Pushes wheelchairs, recliners with residents on them, weighing up to 250 pounds.
  • Assist weight-bearing residents weighing up to 200 pounds, from bed to BSC and
  • to chair on a daily basis.
  • Exerts considerable force in pulling and pushing daily.
  • Hearing ability must be sharp to understand residents, and their needs.
  • Visual ability must be sharp to observe changes in residents' condition.
  • Ability to think clearly, and react calmly during emergency situations, and assist
  • with possible evacuation of residents. RELATIONS WITH OTHERS:
  • Maintain cordial relations with residents, their families, physicians, contracted service providers, etc.
  • Communicates clearly with staff and maintains cooperative spirit
  • LEARNING PERIOD REQUIRED: 1. Six to ten days and then on going. Length of orientation is determined according to the Nursing Staff Orientation Policy and Procedure. WORK ENVIRONMENT:
  • Indoors - works in well lighted and ventilated areas. Subject to possible
  • falls, odors, and injuries from equipment. May be exposed to infectious waste, diseases, chemicals, and various viruses, including Hepatitis B and AIDS. 2.
