The Hebrew Home at Riverdale by RiverSpring Living, a world-renowned long-term care organization located in Riverdale - North Bronx, serves over 18,000 people every day, both in the greater New York community and on our beautiful 32-acre overlooking the Hudson River. RiverSpring Living provides a full range of care solutions including nursing home care, managed long term care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care. RiverSpring Certified Home Health Agency provides skilled home services, including nursing, physical therapy, occupational therapy, speech therapy, and social work. Ideal candidate will be eligible for a $15,000 (less taxes) sign on bonus.ResponsibilitiesKey Responsibilities: As a Home Care RN, you will be responsible for conducting clinical assessments and developing, overseeing, and implementing patient care plans in collaboration with patients, families, and physicians to ensure safe and effective care at home. You will also monitor and adjust care plans as necessary, provide oversight of Home Health Aide services, and participate in the interdisciplinary care delivery process. Coverage Areas: Bronx, Lower Westchester, or Manhattan. QualificationsQualifications:
  • Current NYS RN License and registration
  • Bachelor of Science in Nursing
  • Minimum of 2 years of experience in assessing patients for home care safety and developing care plans
  • Minimum of 1 year of supervisory experience in a hospital or home care agency
  • Patient Review Instrument (PRI) assessment experience preferred
  • Valid driver's license and proficiency in MS Office required
  • Bilingual skills are a plus
Compensation:
  • Annual salary: $105,000 - $110,000
  • Eligible for a $15,000 sign-on bonus (less taxes)
