Position Information:Salary/rate: $22.19/hour Location: Westmoreland County Position Description: The Community Living Supervisor provides a unique opportunity to become part of Achievas management team. This role coordinates the daily operation of assigned Community Home(s). Ensures compliance with Chapter 6400 Regulations; the Allegheny Department of Human Services/Office of Intellectual Disabilities; and Westmoreland County Behavioral Health and Developmental Services standards for the health safety and welfare of the people supported by direct care staff in the home. This position works with assigned staff to support people's outcomes. Together with each person, their family, and other team members, assist in charting a course that supports each person in realizing their life of personal significance. The Supervisor also serves as the on-call person according to a developed schedule, and reports to the Community Living Specialist. Job Responsibilities-Highlights:
  • Supports the achievement of peoples personal outcomes.
  • Respects each person's right to make choices and exercise self-determination, resulting in life experiences and personal growth, supporting their health and safety, and their right to risk.
  • Ensures that opportunities are provided for each person to explore their interests in their community and form relationships.
  • Ensures the Organization,s vision and mission statement and the Department goals and objectives are incorporated in the daily operation of the home.
  • Ensures compliance of the Departments Policies and Procedures relative to Community Homes. Assembles, maintains and monitors all in-house and persons records. Assists with and ensures the upkeep and maintenance of the home.
  • Ensures each person's daily routine appropriately supports that person; including personal care, appointments, leisure activities, and transportation.
  • Assign and ensure completion of each persons Annual Assessment and supporting documents.
  • Ensures the implementation of the outcomes identified in the Individual Support Plan.
  • Ensures effective communication with the people supported, their families and other team members.
  • Assigns direct support staff members to specific job responsibilities at the home.
  • Remains available to assist either by phone or on-site in the event of an emergency, supporting staff and people.
  • Develops the weekly staff schedule. Adjusts staff schedules due to vacation, absenteeism or illness with prior approval. Ensures all shifts are covered.
  • Must obtain CPR certification within six months of the start date.
  • Participates in various committees or workgroups as approved.
  • Acts as Practicum Observer per the Medication Administration requirements.

Supervisory duties can include ensuring appropriate and accurate expenditures and reconciliation of monies dispersed from the Resident account; supervising and evaluating staff assigned to the home(s), coaching on a consistent basis to clarify job responsibilities, program purposes, philosophy, and goals, as well as providing ongoing training, development, and constructive feedback, and implementing correction when needed with prior approval. QualificationsMinimum Requirements: Associate's degree in the human services or related field, and one or more years of related experience working with people with intellectual disabilities OR two years of experience as a Direct Support Professional with one year or more supervisory experience. Good interpersonal and organizational skills as well as strong written and verbal communication skills are needed to be successful in this role. The ability to complete one person transfers is essential. Must meet Act 33 and 34 Requirements and FBI background check. Reliable transportation and a valid driver's license is needed for success in this role. Must obtain First Aid/CPR certification within six months of start date. In addition to paying for federal and state clearances, Achieva offers 24 hours of training, annually, relevant to Human Services. In addition, we have implemented Medication Administration training, which would need to be completed successfully. This training assists our Community Living Professionals in ensuring all prescribed medication is dispensed and documented per P&P in partnership with the individual.
