Job Details- Clinic RN / Larchmont Office at White Plains Hospital
- Job requires the ability to perform clinical/technical/service/administrative tasks
- Responsibilities include assessing patient physical, psychosocial and spiritual health status to determine healthcare and educational needs
- Providing and/or delegating patient care according to care goals and setting priorities based on patient needs
- Assists with triage of sick and walk-in patients
- Establishes therapeutic and supportive relationships with patients and family
- Acts as a resource person and coordinates client care, including referral back to primary provider
- Involves in telephone triage, handling prescription requests, and communicating test results
- Performs therapeutic treatments and assists with procedures
- Administers medications and coordinates all patient care activities
- Coordination of in-patient admissions and ER referrals
- Advocacy for patient needs and ensures patient safety
- Participation in performance improvement activities and educational presentations
- Works as a member of the patient care team and engages in daily huddles and practice team meetings
- Attends in-service and continuing education programs
- Displays professionalism and courteous demeanor with patients and team
- Informs and educates patients/families for participatory care
- Evaluates intervention effectiveness and patient responses
- Collaborates with team for patient care adjustments
- Communicates patient status changes to care team and health care providers
Job Requirements- BLS certification
- COVID Vaccine required
- BSN required
- Minimum $1.25 an hour for parking
Schedule Information- Variable: Mon-Friday 8am-4pm, 9am-5pm, 10am-6pm
Additional Details- FLU & COVID VACCINES REQUIRED - NO EXEMPTIONS
- Ensures all staff participates in patient care
- Effectively communicates with patients, families, and team for care coordination
