The FEP Team offers young people an array of services, including low-dose medication management, Cognitive Behavioral Treatment for Psychosis, Family Education, Case Management, Supported Education and Employment, and peer support services. Service coordination is guided by the young person's voice and choice. FEP seeks to improve the quality of life of young people by instilling hope through empowerment to guide their own treatment, educating them about their psychosis, re-establishing relationships, and re-integrating them back into the community whether it's attending school or working. This position within Delaware County is responsible for developing, coordinating, and implementing services for consumers as identified in the service plan. Responsibilities include providing case management, service planning, and coordination of services with other agency providers to meet the client's needs along with supportive counseling. This position will also provide advocacy to consumers in order to help consumers set and achieve recovery goals and empower consumers to increase self-advocacy, as well as providing supported employment and education services, ongoing job coaching and support for clients who obtain job and school placement. This candidate will work on a closely-coordinated multidisciplinary team to provide community-based support to 15-30 year olds who are experiencing their first episode of psychosis. Program Specific Responsibilities:
- Assist in the completion of comprehensive assessments and treatment plans for each individual client
- Complete a career and education inventory to obtain strengths, needs, aspirations, and barriers
- Develop and implement a written service plan for each consumer assigned to the caseload
- Provide services that diffuse or resolve crises in order to prevent unnecessary use of emergency rooms, in-patient services and mental health court
- Make home, school, hospital and community-based visits at a minimum of every 30 days (or more often as determined by the consumer's level of care)
- Familiar with and able to access community resources and child and transitional age serving systems in Delaware County
- Substantial local travel in your own vehicle to off site locations as required by caseload, supervision and/or training requirements.
- Transport clients and families to various appointments based on caseload as needed
- Document supervision/case activity through ETO, progress notes and other program specific paperwork by typing or handwriting
- Ensure that program files are organized and paperwork is filed into client specific charts
- Substantial local travel in your own vehicle to off site locations as required by caseload, supervision and/or training requirements
- Transport clients and families to various appointments based on caseload as needed
- Achieve billable hour requirements per week
- Assume case load of up to 30 clients
Required Qualifications:
- Bachelor's degree in Human Service or related field
- One or more years of direct clinical experience working with children, youth and families
- Current child abuse, criminal and FBI clearances
- Maintain valid driver's license and insurance
Preferred Qualifications:
- Bi-lingual in Spanish
- Experience with job coaching
- Experience working with Transition Aged Youth
- Knowledge of resources in Delaware and surrounding counties
- Experience with crisis management
- Experience working with Schizophrenia Spectrum
Work Environment:
- Work in standard office environment as well as community settings to include client home, schools, court, doctor's offices and hospitals.
- Evenings and weekends may be required
