Job Description :
Robynwood Assisted Living is seeking a Registered Nurse Supervisor / Case Manager to join our team! As a Registered Nurse Supervisor / Case Manager, you will:
  • Be responsible for the direction/supervision of all client case services, and other health care activities of the agency.
  • Manage the development and/or implementation of care plans within the scope of practice.
  • Implement standards, maintain integrity of operational policies, and participate in performance improvement activities.
Additional Responsibilities:
  • Interprets standards/policies/procedures to ensure compliance with external regulatory authorities.
  • Works with physicians, family members, and other care providers necessary to implement and establish a care management profile.
  • Records all client progress and reports to all contractees as required.
  • Handles individual emergencies, seeking our appropriate assistance when necessary.
  • Attends facility and program related meetings.
  • Participates in requiting, interviewing, selection and orientation of team members.
  • Evaluates team members' performance relative to job goals/requirements.
  • Assists/over sees new employee training and education in-service programs.
  • Serves on the Quality Assurance Committee, reviews and revises all written policies and procedures, and ensures adherences to internal policies/procedures.
  • Assists with resident admission to the Adult Home and Assisted Living Program.
  • Allows client/resident individual access to his/her case records.
  • Provides information, and refers clients to other community services as needed.
  • Achieves, to the maximum extent possible, economic efficiencies, including but not limited to, using shared aides consistent with the A.L.P.'s staffing standards.
  • Directly supervises self administration of medications or assists with medications as assigned.
  • Conducts medication audits.
  • Problem solves, and communicates needs to ambulance service, physician, family, and administrator as appropriate.
  • Ensures that all medications are ordered, received, taken, documented, stored, and disposed of in accordance with Robynwood Policies and Procedures.
  • Delivers, assists with, and supervises treatments.
  • Assists Administration in overseeing delivery of services specified in the individual care plan.
  • Ensures that the confidentiality of all resident information is maintained and that the privacy is respected.
  • Serves as a role model and coach for all staff by demonstrating approaches and techniques that enable residents to do for themselves for as long as possible and manage challenging behaviors.
  • Ensures that residents are free from abuse, neglect, and exploitation and reports such instances to the appropriate agency and Administrator after investigating the incident.
  • Communicates resident changes in condition to family
  • Implements and monitors cost containment measures without jeopardizing resident care.
  • Ensures adequate staffing coverage to perform resident care.
  • Provides constructive feedback to staff in a specific and timely manner.
  • Ensures that staff comply with state regulations related to resident rights
  • Coordinates, implements, and instructs others on the application of clinical protocols developed to manage resident care such as bowel and bladder management, nutritional status, and incident reports.
  • Monitors infection Control among employees.
  • Communicates to other departments to ensure on-going maintenance and advocates employees and residents needs.
  • Participates, as appropriate, in state surveys made by NY authorized agencies and ensures compliance with regulations.
  • Assists Administrator and Training Coordinator in orienting and training of new staff and documents all training
  • Attends and participates in continuing education
  • Conducts / Attends on shift, or other meetings as requested
  • Oversees building in absence of other management staff
  • Assists in solving family issues and reports to Administration
  • Conducts case management in correlation with other shift supervisors
  • Alerts physicians in the event of significant medical status change
  • Follows pharmacy and supply delivery check-in procedures
  • Serves on the Risk Management Team
  • Maintains Incident Report Records, follows policies and regulations in accordance to incident reporting
  • Provides information and applications to potential residents and families
  • Identifies potential residents and works with area agencies to place the individual at Robynwood
  • Assists with developing and maintaining teams
  • Provides in-service education as needed or delegated by the Administrator or training Coordinator.
  • Monitors and provides documentation of changes in resident condition and services including follow-up documentation.
  • Directs and Oversees Care giving Staff, discussed behaviors with staff appropriately utilizing a developmental feedback or recognition Form.
  • Seeks guidance/ or assistance from Administration when needed.
