JOB SUMMARY

The Social Worker provides social work services for residents and families under the general direction of the Director of Social Services. The Social Worker also serves as a liaison between the interdisciplinary team, residents, families, and appropriate outside agencies. On occasion, the Social Worker may be required to drive a vehicle in order to make home and community visits or make shopping trips for residents.

QUALIFICATIONS

Education & Training: Graduation from an accredited four-year college or university with a Bachelor's Degree in Social Work or a related field required. Two (2) years of satisfactory paid social casework experience in a healthcare setting preferred.Special/Other: Successful candidates will enjoy working with and helping people. You will organize your work effectively, get along well with others, and exhibit effective communication and listening skills. You should have a working knowledge of psychosocial factors related to disease and disability as well as social casework practices and procedures. You must understand medical and social work terminology.JOB DUTIES

  • Participate in pre-admission interviews and assist with admission paperwork and orientation;
  • Obtain initial psychosocial history upon admissions, identify psychosocial needs and problems as they relate to resident medical condition(s), establish goals, and facilitate adjustment to the long-term care environment;
  • Develop and maintain a trusting relationship with residents and assume an advocacy role;
  • Provide counseling, support, and direct services to residents and families, including referrals to appropriate community agencies, etc.;
  • Facilitate interdisciplinary care plan meetings;
  • Assist families or residents as needed with issues of finance including personal needs allowances, Medicaid applications, and annual recertifications;
  • Interact with community agencies as necessary;
  • Participate in resident council, communicate council concerns to appropriate department(s), and compose minutes of meetings;
  • Develop/coordinate individual discharge plans with staff, residents, family, and community resources;
  • Maintain positive working relationships with community partners and collect and update information on community health and welfare resources;
  • Interview residents and responsible party(ies) to obtain initial social history and identify barriers as they relate to the overall resident care plan;
  • Document resident status and discharge planning activities, as appropriate;
  • Ensure social and personal aspects of resident care are adequately and properly managed;
  • Maintain knowledged of all current and applicable state and federal regulations. Assist in the development and interpretation of facility policies that relate to these regulations and educate residents and staff regarding these policies;
  • Complete required social work documentation;
  • Perform other related duties as requested.
