The organization is dedicated to the community and focused on providing resources and opportunities. East Side House has identified that education is the foundation for economic improvement. Our youth education and technology programs teach skills to allow students and clients to become more self-sufficient and independent. As a community resource, the organization also provides services to families and a growing senior population. In 2020 the organization began food distribution as an additional way to provide economic support to the community we serve. Jobs-Plus is a proven, place-based employment program designed to increase the earnings and employment of working-age residents in designated public housing developments or a cluster of developments. Jobs-Plus - Mill Brook Houses, is operated by East Side House Settlement and based in the Mott Haven neighborhood of the South Bronx. It offers workforce development, financial empowerment services and community building programming to over 3000 residents. With a recent 3-year contract renewal, Jobs-Plus is looking to expand on the services provided to residents since 2013. This expansion will include heightened focus on training and education as well as career advancement services. The Clinical and Wellness Coordinator (CWC) will report to the Assistant Director of Jobs Plus. The CWC will provide onsite resources to Jobs-Plus members to assist with addressing and overcoming mental health barriers preventing goal attainment. A successful candidate will display the following characteristics:
  • Commitment to mission and goals of East Side House Settlement and Jobs-Plus.
  • Ability to collaborate with colleagues, including onsite Managers of service teams.
  • Passion and interest in providing direct case management to members.
  • Skills necessary to coordinate and lead case management conferences and respond to needs of clients/staff.
  • Flexibility and organization necessary to report to co-supervisors.
  • Ability to identify, plan and lead staff trainings in socio-emotional topics to improve services provision.
  • Strong attention to detail and follow-through.
  • Relationship with local resources, stakeholders, community leaders, etc.
Primary Functions Duties and Responsibilities include, but are not limited to, the following:
  • Lead integration of C2C in the Jobs-Plus program, including coordinating staff trainings and continuing coaching as needed, leading technical assistance engagement of the site, participating in Case Conferences, coordinating pathways to clinical care for members, and other related functions.
  • Engage with HRA technical assistance provider and Clinician, using qualitative and quantitative data, to identify mental health needs and priorities, characteristics of Jobs-Plus population served, and patterns in service use or referral needs.
  • Develop plan for interventions and will include, but not be limited to, gathering information from program leaders, staff, and members.
  • Coordinate initial training for and/or provide training to staff in the selected interventions and in the C2C model, based on the C2C plan.
  • Use validated tools to screen consenting members for mental health symptoms and re-screen members at appropriate intervals (such as PHQ9 and may vary, based on population needs and the interventions selected) to identify changes in symptoms and adjust clinical services.
  • For psychoeducation, continually implement at least two selected strategies per year through use of one-on-one sessions, group sessions, or one-on-one conversations, based on the evidence-based intervention selected.
  • Provide counseling services for Jobs-Plus members, on site at the Jobs-Plus program at least 2-3 days a week in collaboration with Clinician. On-site services would be delivered in a manner that ensures privacy and confidentiality for those receiving services.
  • Host onsite events and workshops in the area of mental health and wellness awareness.
  • Continue to adapt and expand interventions that are used, with HRA approval, including training staff in new interventions.
  • Facilitate the appropriate referrals for clients who need a higher level of care.

Expected Work Schedule This role is a full-time position expected to generally work Monday through Friday, 9 a.m. to 5 p.m. and rotation of first Saturday of every month (10-3PM). Workdays/hours subject to change based on ESH's needs. Required Clearances COVID Vaccine - As a recipient of Federal, State, and local funds, all staff are expected to be fully vaccinated or able to obtain vaccination before their start date Supervisory Responsibility None. Work Environment Business casual. This job operates in a school office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Travel Minimal expected for this position. While performing the duties of this job, the employee is regularly required to talk or hear. This role is generally a sedentary role; however, some sorting/filing is required. Requirements include the ability to lift files, open filing cabinets and bend or stand on a stool as necessary. QualificationsRequired Skills/Experience
  • Bachelor's degree and must have a minimum of five (5) years of successful experience working in social services, mental health and/or substance use, or a related area
  • Ideal candidates have experience implementing partnerships and experience in the implementation of community mental health service models, though these are not requirements.
  • Bilingual Spanish preferred
  • Excellent verbal and written communication skills
  • Ability to work with a culturally diverse population
