Design, implement and monitos individual program plans for clients. Assess client needs; investigate their eligibility for services. Counsel and aid individuals and families requiring agency assistance. Maintain client records and complete required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs. Coordinate client activities. Assist clients with accessing other services (job training, counseling, housing, medical services, etc.) Network with other providers to generate community resources for participants. May provide supportive counseling in individual and group settings. May transport consumers to appointments, etc.PROGRAM RESPONSIBILITIES:
- Conduct PALS program intake and assist in the orientation of clients
- Ensure standardized assessments are completed by PALS parents pre and post treatment
- Maintain weekly contact with PALS parents
- Research and maintain database of current case management resources
- Facilitate biweekly case conference meetings with PALS staff
- Coordinate client scheduling
- Assist Program Coordinator in the day-to-day administrative running of the program
- Assist with child care and client transportation, as needed
OTHER DUTIES: As assignedPosition Requirements MINIMUM QUALIFICATIONS: Bachelor's degree in social work, psychology, sociology, guidance and counseling, child development, criminal justice or education. Related work experience preferred. 40-hour domestic violence training required.LICENSES/CERTIFICATIONS NEEDED: Valid driver's license required.MANDATED PROGRAM REQUIREMENTS: Providence House is a 24-hour residential program, as such requires staffing at all times.
