Works with chronically homeless Veterans, mentally ill, and dually diagnosed individuals in transitional shelter program. Provides an overall support system for client caseload and refers them to local agencies and VA centers for vocational, educational, and health and human services. Essential Duties and Responsibilities: * Assesses clients needs and appropriate services and assists in implementing service plans. Provide the VA HCHV staff an individualized services plan within three days of admission and discharge summary within three days of discharge. * Maintains an active caseload of clients who need intensive case management and assists with the Program Managers caseload in their absence. * Provides a wide variety of client services such as: counseling; providing referrals and resources; providing life skills training to clients such as managing a budget, diet, health maintenance, legal issues, and medication monitoring; and ensuring clients are safe and secure. Applies for entitlement benefits and arranges transportation services for clients to scheduled medical and/or social services appointments. * Monitors client progress in substance use recovery and mental health status; collaborates with treatment providers to address crises, relapse and harm reduction strategies as needed. * Participates in meetings with service providers including physicians/psychiatrists to help resolve conflict and to monitor progress. * Coordinates prescribed medications for clients including arranging refills with pharmacy, picking up refilled and new medications, addressing pharmacy coverage issues with insurance, and assisting clients in filling medication trays. * Maintains client files and active resource files. * Provides documentation to the Program Manager for submission to VA HCHV of services provided to each Veteran monthly, outlining quality and effectiveness of services provided no later than the eighth day of each month. * Collaborates with the VA Safe Havens Program Manager, Veteran, and HCHV staff to conduct treatment and discharge planning reflecting a team assessment of health, social and vocational needs, and the involvement of Veteran's family and appropriate community resources in resolving problems and setting goals. * Completes psychosocial assessment and maintains all pertinent client paperwork. Also completes client data for reporting purposes including accurate and complete data in HMIS - Service Point. * Complies with VA reporting and data collection system requirements. * Coordinates for Veterans to work towards transitional or permanent housing within six (6) months of admission. All extensions beyond six (6) months will require approval of the VA. * Supervises House Manager including completing ongoing performance plans as needed and administering annual performance evaluations. Assists House Manager with operational tasks and staff coverage issues as needed. * Provides guidance to House Manager and staff on clinical issues. * Assesses clients in crisis and resolves conflicts. Develops interventions to resolve problematic behavior of clients. * Co-facilitates with House Manager regular staff and house meetings. * Participates in weekly peer supervision with other Safe Havens Case Managers, Program Manager, and Senior Program Officer. * Acts as representative of MCCH in collaboration with other service providers. * Other related duties as assigned. Requirements * Masters Degree Licensed Professional. Minimum of three years of experience in social services or related field. * Hands-on experience in social and human services field dealing with diverse populations. * Knowledge of principles, methods, and procedures of case management. * Knowledge of principles, methods, and procedures in working with addiction and dually diagnosis populations. * Ability to negotiate and maintain positive relationships with co-workers and clients. * Advocate for homeless population.
