Overview Schedule: 7:30am - 4:00pm (Monday - Friday) Purpose Statement/Position Summary: The Care Coordination Assistant plays a pivotal role in the healthcare system by facilitating the effective and efficient use of medical resources while ensuring high-quality patient care. Under general supervision, will follow general procedures and perform a wide variety of clerical duties that support patient discharge. The Care Coordinaton Assistant in Rehab Support will provide administrative support to two Rehab Coordination Managers in the inpatient Rehabilitation unit in order to ensure timely processing of admission requests into the unit, maintain appropriate approval throughout the stay, and support discharge referrals for outpatient follow up. The team is responsible for processing and scheduling patient referrals sent to the Rehab outpatient clinic. Minimum Qualifications/Work Experience: One year secretarial or unit assistant experience, or equivalent combination of education and experience. One year experience in a medical setting; experience in medical transcription or equivalent training; experience working in a case management setting; experience in front office medical setting experience highly preferred. Scheduling experience strongly preferred. Knowledge of medical insurance highly preferred. Education/Licensure/Certifications: High School diploma or equivalent required. College experience preferred. Training or experience in health care field, familiar with health care terminology. Training or experience with basic secretarial functions/equipment (PC, fax, telephone, copier). Effective verbal and written communication skills. Broad knowledge of computer software including word processing, spreadsheets and presentation applications; keyboarding skill level at 45wpm. Preferred for positions in medical/clinical areas: basic medical and scientific terminology. Bilingual preferred. [If fluent in another language, assist in interpreting on a limited basis (phone calls to families to check demo info, brief dc planning on phone, discuss learning needs/expectations). NOT for extended teaching, not to translate DC orders/instruction in KIDS.]. Pay Scale Information $46,800.00-$80,850.00 CHLA values the contribution each Team Member brings to our organization. Final determination of a successful candidate's starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
