The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Position Summary

Under the direction of a physician or senior nursing staff, the Clinical Coordinator I (Registered Nurse I), assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care and clinical protocols as needed. These duties include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator I (Registered Nurse I) will work closely with multiple providers as well as other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.

Responsibilities

Essential Duties * Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician. * Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed. Administer immunizations and medications. * Under supervision, executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department. * Supports on-going clinical functions of the service to provide compassionate patient-centered care. * Handle telephone, My Chart and in-person triage (for symptom management) as well as patient education under supervision as appropriate. * Assist with management of urgent and emergent patient situations in conjunction with care team leadership. * Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed under physician supervision. * Ensures infection control compliance with in the practice. * Addresses appropriate measures to provide patient and family satisfaction. * Provides coverage at the departmental practice sites as needed. * Other duties as assigned and appropriate to scope, education, and/or training.

Minimum Qualifications

* Requires a bachelor's degree in Nursing or equivalent in education and experience * 0-2 years related experience. Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements. * BLS certified * Current New York State RN License * Ability to perform sedentary work that primarily involves sitting or standing for long periods of time * Travel between clinical sites is required depending on need * Duties performed routinely require exposure to blood, bodily fluid, and tissue. Incumbent in expected to perform and/or/assist in emergency care and/or first aid as needed * Ability to work collaboratively with a culturally diverse staff and patient/family population demonstrating tact, respect, and sensitivity. * Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management and other stakeholders. Must have highly developed interpersonal skills and ability to deescalate stressful situations. * Must be an engaged and positive team member demonstrating collaboration, inclusion, reliability, punctuality, and adaptability to new and changing situations including helping peers adapt to new situations. * Strong proficiency in computer skills including, but not limited to, Electronic Health Systems, Microsoft Excel, Microsoft Word, Email systems, etc.

Preferred Qualifications

  • OB/GYN practice experience preferred.
  • Familiarity with EPIC a plus.
