**Employment Type:**Full time**Shift:**Day Shift**Description:****Registered** **Nurse-Liaison-Hospice-Intake-Albany**The Community Hospice has a rewarding opportunity for a RN to work as a part of our Hospice Intake RN Team serving as Liaison in local hospitals (Capital Region-primarily St Peter's Hospital, Albany NY).**St. Peter's Health Partners Highlights:**+ **Mileage Reimbursement**+ **Quality of Life:** Where career opportunities and quality of life converge+ **Advancement:** Strong orientation program, generous tuition allowance and career development+ **Work/Life:** Positions and shifts to accommodate all schedules**What you will do:**+ Evaluate patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care.+ Coordinate with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.+ Gather information and ensure a smooth handoff to the hospice RN admitting the patient/family to services.+ Work closely with the hospital case management department.+ The work schedule is flexible, generally weekdays (M-F) between 8am to 4pm. Weekend opportunities may also exist from time to time.**What you will need:**+ A current license to practice as a Registered Nurse in the State of New York+ Associates Degree in Nursing, BSN preferred+ 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.+ Hospice or RN Liaison experience a plus but not required.+ The RN must be able to communicate effectively, verbally and written+ Valid Driver's License and Reliable vehicle+ High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers+ Understanding of Hospice principles preferredTraining provided in end of life care.Community Hospice provides a thorough orientation, preceptors and a supportive work environment.Apply today for more information._Pay Range: $33.10-$45.75_ _Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
