Job Summary

PACE Lafayette (Program of All-Inclusive Care for the Elderly) is a nonprofit health and supportive services program designed to assist seniors who want an alternative to long-term care placement. PACE is sponsored by the Franciscan Missionaries of Our Lady Health System (FMOLHS)-the parent company of Our Lady of the Lake Regional Medical Center-the Centers. The mission of PACE Lafayette is to enable the aging population to live with dignity in their communities. With PACE's assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance, physical relief and emotional support.

Our team works together to develop individual care plans, goals and carefully monitors progress. Our multi-skilled team-consisting of a doctor, registered nurse, social worker, physical therapist, occupational therapist and registered dietitian-works together to develop patient goals, plan care and carefully monitor progress.

Job Summary

In the participant's home or at the Center, performs Participant Care Activities and Personal Care and other tasks as assigned to assist the nursing staff.

Minimum Requirements

Education: HS Diploma or Equivalent

Experience: One year of C.N.A. or Personal Care experience working with frail/elderly population

Training: DSW class or CNA course involving training with care of elderly

Special skills: Must meet all qualifications of a FMOLHS approved driver

Licensure: DSW or C.N.A license

* Patient Care: This will have been satisfactorily performed when...

* Recording pertinent information pertaining to the participant's care.

* Reminding participants to take medication at the appropriate time.

* Performs meal functions which can include planning and preparing meals according to specific dietary requirements of the participants, if necessary, oral feeding of the participant, serving of meal, supplements and snacks, sanitizing dishes, utensils and serving pans, following guidelines of food storage.

* Assisting the participant with therapy exercises under the direction of the Physical/Occupational Therapist.

* Assisting participants with basis Activities of Daily Living (ADL) care of designation participants including but not limited to transferring, bathing, dressing, catheter care, basic skin care, incontinent care, toileting, and vital signs as assigned.

* Assisting with ambulation's with or without mechanical aids.

* Maintaining a safe, clean and healthy environment through light housekeeping, including changing bed lines, dusting and vacuuming, cleaning kitchen, bathroom and doing laundry.

* Providing companionship and stimulation for the participant including reading, walks, etc.; and accompanying the participants to doctors or other appointments.

* Independently or in the company of participant, perform grocery shopping or other errands per Care Plan.

* Take participant to medical or other appointments as indicated in Care Plan.

* Performing other housekeeping tasks as indicated in the Care Plan.

* Observing the reporting changes in the participant's condition, incidents or any other job - related problems to the Supervisor and communicates participant changes to team members.

* Always follow Participant Rights guidelines including confidentiality to ensure participants are being treated with dignity and respect as adults. Communicates effectively with participants, families, staff, other departments and agencies.

* Accurately prepares daily records and submits them to the office by the due date.

* Participating in in-service education programs is mandatory.

* Complete other tasks as assigned by supervisor.

* In the Center, assists in the maintenance of the Adult Day Center to provide for a pleasant, efficient, and safe environment. Maintains laundry and disinfecting of Adult Day Center on a routine basis. Maintains clear pathways for participants and assists with processes when participants arrive and depart on the transportation van. Performs environmental rounds as scheduled.

* Knowledge of unit-based quality improvement initiatives is always demonstrated.

* Participant care is documented in the Electronic Health Record or the appropriate home care documentation daily.

* Instructions issued by the PACE Interdisciplinary Team to resident care routines and policies are always followed.

* Other Duties as Assigned: This will have been satisfactorily performed when...

* Other duties as assigned are completed.

* Active participation on unit teams, work groups and/or committees is provided as requested; anticipates needs and voluntarily assists others.

* Attends all mandatory in -services required by CMS regulations.
