The Southeast Permanente Medical Group, (TSPMG)/ Kaiser Permanente is happy to announce an opening for a POOL Advanced Medical Care At-Home Intake Coordinator. This individual will be responsible for clinical processes, coordination, and support for the Advanced Medical Care At-Home (AMCAH) program. They will also manage AMCAH admissions and coordinate services of admitted patients, serve as clinical liaison between AMCAH patients and physicians and serve as the first point of contact for patients and vendors. The ideal candidate will have 3 years' experience as a RN (current, unrestricted GA license), BSN, BCLS certification, 2 years' experience in hospital nursing, experience in referral intake and proficient computer skills.
Major Responsibilities/Essential Functions: - Verifies global eligibility criteria, admits patients into AMCAH, coordinates discharge needs and hand-offs to appropriate department, and discharges patients from the program
- Implements physician orders including coordination of vendor services
- Creates patient schedules for clinical team
- Responds to member/family concerns, clinical and non-clinical
- Assigns equipment to vendor partners needed for visits and secures return of equipment when appropriate
- Coordinate and facilitate huddles and meetings with clinical teams, vendor, key stakeholders, or project teams; drive the groups to decisions and resolutions.
