Job Summary:
The Community Health Worker's role serves as the liaison between health care providers, care coordination, and community engagement to facilitate service delivery within identified communities. Some occasional night or weekend work.
EXPERIENCE DESCRIPTION:
A minimum of 1 year of experience working with diverse communities coordinating and facilitating service delivery.
EDUCATION DESCRIPTION:
High School diploma or equivalent is preferred.
TRAINING DESCRIPTION:
Completion of Community Health Worker Program through accredited organization (Sponsored by Froedtert Health if no prior training upon hire. Must Be Completed within One Year of Employment at Froedtert Health).
