Now hiring for a full-time RN Case Manager for our Assisted Living/Adult Care Facility.
Essential Job Functions Include:
- Provide each Resident with case management services as needed to promote independence and freedom of choice.
- Orient new Residents and their families to the facility and their new routines. Encourage Resident participation in facility and community activities.
- Act as a liaison for the client and family with the health care team/community agencies. Organize/coordinate any transportation needed for appointments.
- Complete medication audits of Residents profile, weekly medication orders and renewing needed PCP orders.
- Establish linkages with and arranging for services from public and private sources for income, health, mental health and social services. Assist Residents in applying for income entitlements and public benefits.
- Observe Residents for changes in condition, address as appropriate. Be available to participate in client hands-on care and comprehensive care planning.
- Provide support and leadership to HHAs and PCAs as needed. Identify and delegate tasks to be completed and monitor staff compliance with safety standards.
- UAS completed every 6 months and, at least once every 12 months, of the needs of each Resident and of the capability of the facility program to meet those needs.
- Maintain professional working relationship with contracted CHHA.
- Assume on-call rotation as needed.
- Additional duties as assigned by the Assisted Living Administrator.
Requirements:
New York State Registered Nurse.
Qualifications:
- Geriatric nursing experience preferred.
- ACF/ALP Knowledge preferred.
- Strong organizational and leadership skills.
- Familiar with regulatory agencies.
- Must be available for Holidays, Weekends, or On-Call Duties.
- Must be able to read, write, speak and understand the English language in an understandable manner.
- Must be able to perform essential job functions (reasonable accommodation may apply.)
- Must be able to function independently, have flexibility, personal integrity and the ability to work effectively with Residents, staff, visitors, volunteers, government agencies/personnel, etc. under all conditions and circumstances.
