Job DetailsJob Location: CHC Mashpee - Mashpee, MASalary Range: UndisclosedDescriptionThe Community Health Worker (CHW) is responsible to apply a unique understanding of the experience, language and culture of a targeted population in providing care management support to patients. As a patient navigator, the CHW will work to link patients to community resources and services that will assist and motivate patients to meet health goals.Duties will include:* Weekly follow up calls and/or in person meeting with patients referred from primary care teams.* Assisting patients in filling out applications for eligible programs and services, helping to organize records and necessary documents as well as approval status follow through.* Creating a directory of community resources* Documenting encounters and preparing reports as required, utilizing numerous applications and databases for tracking.Qualifications2 years + experience or commensurate education or training. Strong knowledge of community resources and demonstrated ability to set goals and prioritize workflows. Options available for hybrid remote role.
