Nuvance Health has a network of convenient hospital and outpatient locations - Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York - plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.Summary:A paraprofessional person who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN) in the licensed agency.Responsibilities:1.Understands and adheres to established Agency policies and procedures.2.Performs all PCA health related task activities under the direction and supervision of a registered professional nurse utilizing a written plan of care.* The following are examples of possible routine health related tasks to be performed by the PCA: Bath, change of bed linen, toileting, dressing, feeding, shampooing, shaving, mouth care, nail care, skin care; Ambulation and transfer activities; ostomy care; Reinforce the patient's proper use of assistive devices; Assist with self-administered medication; Take and record pulse, temperature, respiration and blood pressure; Perform light housekeeping activities (tidying, shopping, laundry) as necessary to maintain a healthful environment; Observe, report and record changes in the patient's condition and/or environment to appropriate staff as needed; Use of proper body mechanics; and adhere to accepted infection control practices.3.Completes appropriate visit record documentation in a timely manner as per Agency policy.4.Reports changes in the patient's condition and needs to the RN.5.Performs household services essential to health care in the home as assigned.6.Assists in maintaining or improving the patient's independent functioning as patient condition allows.7.Assists with medications that are ordinarily self-administered as assigned.8.Ensures patient's rights and confidentiality are maintained.9.Establishes a positive rapport with patients, patient's family members and other members of the health care team.10.Follows all departmental and organizational protocols and practices to ensure service excellence is achieved with both internal and external customers, and that boundaries within the scope of practice are not exceeded.11.Attends in-service and continuing education programs as scheduled and necessary.12.Attends patient care conferences as scheduled.13. Demonstrates regular, reliable and predictable attendance.14. Maintain and Model Nuvance Health Foundational Values (Personal, Imaginative, Agile, Connected).15. Performs other duties as required.Other information:Education and Experience Requirements:* Must meet New York State Department of Health regulations for Personal Care Aide training program and competency.Minimum Knowledge, Skills and Abilities Requirements: ·Basic Computer EMR skills.
