Apply today and begin your career by making a difference and fulfilling your sense of purpose!JOB SUMMARYUnder the direction of the Person Supported and Fiscal Intermediary, this position is responsible for providing person directed services and supports as outlined in the person's Self-Directed Budget, Life Plan, and Staff Action Plan(s).ESSENTIAL FUNCTIONSRegulatory and Corporate ComplianceEnsure compliance with regulations, agency policies, and procedures.Ensure required documentation is complete, accurate, and legible.Ensure the agency's incident management process is followed.Provide input in the development and revisions of policies, procedures, and agency forms, as needed.Ensure Chain of Command is followed when necessary and instructions are followed as stated in policy and procedures.Financial ManagementAssist with ensuring adherence to the annual budget.Assist with ensuring accurate management of person's funds.Complete financial documentation according to procedure.Person Centered ServicesProvide services to the person as planned in their Life Plan, which includes the person's Staff Action Plan(s) and/or other safeguards and ensure all related documentation is completed.Act as an advocate for the person.Provide person directed services in personal skill areas.Complete assessments and compile baseline data as assigned.Assist and support person with community integration.Respect the person's right to privacy and confidentiality by appropriate use of written and verbal communication.Work collaboratively with others to ensure the person's health and well-being.Report and document all person's medical concerns to the appropriate personnel.Attend health and medical appointments as assigned.Maintain on-going communication with the Fiscal IntermediaryProgram StandardMaintain an environment which encourages and facilitates optimal personal development.Complete assigned daily responsibilities.Assist in the identification and development of new services and support opportunities that provide quality of life choices.Complete documentation and monthly summaries as assigned, if applicable.Supplies are the responsibility of the person/family.The use of the EVV (electronic visit verification) will required.Physical Plant and SafetyEnsure a safe, clean environment, and immediately address areas of concern.Practice standard precautions and other appropriate infection control practices.Contacts with OthersDevelop and maintain positive relationships both internal and external.
