Description We are seeking an experienced MDS Coordinator to join our team! Come and join our CARE culture and be part of a team that values the lives of our residents and community. The MDS/Care Plan Coordinator is to complete the RAI process, for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State. Also, maintain compliance with our own policies and procedures. Ensure that the highest degree of quality care is always maintained. As the MDS/Care plan Coordinator, you are delegated the administrative authority, responsibility, accountability necessary to carry out your assigned duties. Requirements Responsibilities:
- Interpret the facility's policies and procedures to employees, residents, family members, visitors, government agencies, etc. as necessary.
- Complete the RAI process timely, for each resident in the facility.
- Conduct the Care plan meeting and formalize the written care plan utilizing the information obtained at the meeting. Include the resident and family.
- Review charts to ensure that they are informative and descriptive of the nursing care provided and that they reflect the residents' response to nursing care as it relates to the care plan. Report problem areas to the Director of Nursing Administration.
Qualifications:
- Valid Texas Nurse License
- Valid CPR Certification
- Minimum of one year of experience in a supervisory capacity in a hospital, skilled nursing facility, or other related healthcare program
- Must possess knowledge of long-term care operational standards as set forth in the Federal Register, Conditions of Participation and State regulations
