Position overviews:

• The RN Case Manager is responsible for facilitating the patient s hospitalization from preadmission through discharge from the ED to the community or to an alternative level of care.
• The RN Care Coordinator is responsible for ensuring that utilization of resources including medical services, length of stay and appropriate status class.

Requirements: Five years of acute hospital case management/care coordinator experience

License/Certifications: CA RN license; Preferred Case Management Certified
