  • Salary: $20.66 - $29.49 per hour

Actual compensation offered will vary based on factors including applicable licenses, certifications, education, experience, location, qualifications and comparison with associates in similar roles.

Schedules with guaranteed hours will include hours every other weekend and some holidays; Potential to float.

Responsibilities

As our LPN at Our Lady of Peace, you will make an enormous impact and form strong connections with your patients and their families!

As an associate with Ascension Living, you'll join a team of professionals dedicated to person-centered, high-quality, compassionate care. As a valued member of our care team, you'll work together to provide basic bedside care to long term care residents under the direction of a Registered Nurse (RN) or physician, functioning within scope of practice and:

  • Administers and controls medication and treatments as ordered by physician.
  • The Licensed Practical Nurse, monitors the medical condition of residents and reports changes to the Unit Manager.
  • The Licensed Practical Nurse monitors and ensures accuracy of recording on resident medical records. Maintains all required reports, records, statistics as directed by the Unit Manager.
  • Under the supervision of the Unit Manager, the Licensed Professional Nurse provides direct resident care, evaluates outcomes and adjusts nursing care processes as indicated to ensure optimal resident care.
  • The LPN monitors and supervises the performance and follows the implementation of resident care by the CNA's on his/her unit.
  • The Licensed Practical Nurse assists physicians with examinations, procedures and other processes related to direct resident care as directed.
  • The Licensed Practical Nurse assists physicians, co-workers and families as needed in time of stress or crisis.
  • Enhances professional growth and development within the Nursing Home through participation in educational programs, current literature, inservice meetings, and workshops.
  • Attends all inservices and meetings as required.

The following duties may be reassigned:

  • Orients, instructs, and trains assigned personnel.
  • Ensures an adequate stock of supplies and proper functioning of equipment.
  • Performs other related duties as assigned or requested.
Requirements

Licensure / Certification / Registration:

  • Must possess Basic Life Support Certification credentials, required.
  • Current New York State LPN License, required.

Education:

  • Graduate from an accredited school of Practical Nursing, required.

Work Experience:

  • On the job training provided.
  • One (1) year medical-surgical skilled nursing home experience, preferred.
