Clinical Intern (Patient Navigator): Applications are accepted on a rolling basis.Priority Deadline: November 15th, 2024Location: Hybrid (In-person hours at Jamaica, Flushing, & Woodside in Queens, NYCHours per week: approx. 12-15 hours/week (12 hours per week in patient navigation plus 2-3 hours additional data entry, admin support & charting). Interns will be provided with training to prepare to serve as navigators in the first week of their term. We will prioritize applicants who can serve until June 2025.RequirementsWe are looking for Patient Navigators to perform on-site and telephone-based patient navigation for our Strong Children Wellness Medical Group practices in Flushing and Jamaica, Queens. Interns should be interested in working with marginalized populations, including those affected by poverty, immigration challenges, homelessness and/or child welfare system. These interns will be responsible for:
  • Screening, navigation and referral to community resources for unmet social needs;
  • Arrangement of transport services and subspecialty appointments for patients and
  • Telephone support and text follow-up for patient concerns
  • Data entry into the screening patient database for the practice.
  • Depending on navigator experience, managing a small panel of patients receiving case management
  • Our patient navigators will provide mostly remote and in-person support as needed. They must be able to attend weekly meetings with the lead navigator to report progress throughout the week. Data organization includes working with our EMR and our HIPAA-compliant text messaging service, OhMD, for which training will be provided.Patient Navigators can earn course credit for their work. As desired, there are also additional research activities outside of clinical work, including data collection to support manuscript development and conference presentationsQualifications:
  • Be in good academic standing
  • Must be available for at least two in-person 4 hour shifts/ week (9a-1p OR 1-5p on Tue, Wed, Thurs or Fri). Shifts can be arranged with your class schedule.
  • Have excellent communication skills and attention to detail, demonstrate flexibility, and possess strong teamwork skills
  • Experience working with vulnerable families and diverse communities (e.g., low-income, immigrants, et, preferred but not required.
  • Preferences:
  • Have Spanish language fluency ( Full professional proficiency or native fluency)
  • Experience working with families involved in ACS or the child welfare system,
  • Coursework in public health, a plus.
  • Able to work until June 2025.
