Job Type Full-timeDescriptionBASIC FUNCTION : The Case Manager will conduct initial screening of incoming clients and enroll clients in the Wellness Early Intervention Program. S/he will deliver Case Management services in compliance with CCM protocols. In addition the WEIP Case Manager will conduct Wellness Self-Management sessions and will work closely with LESC's existing Care Coordinator and Health Navigator. The WEIP Case Manger will also provide needed linkages to care for mental health, and primary care services; s/he will initiate clients' EHR and will conduct Rapid HIV testing as requested and refer those positive for confirmatory blood work to LESC's medical unit or to LESC's partner in care Betances.
