Job Description :

Avita Home Health & Hospice is looking for a Home Health Aide (HHA) for patient home and long term care facilities. This person will work with a caseload of patients alongside a RN Case Manager. This position is a little different every day and you will be on the go visiting patients in the community throughout your entire day. Providing an extraordinary patient experience is our top priority. Together with the rest of the team, you will play a vital part in making sure our community thrives and our patients and families receive the best care in the most important time of their lives. Responsibilities:

  • Provide routine patient care in the patient's home or long term care facility
  • Remind clients to take prescribed medication and assist clients with ambulation
  • Assist the patient with daily hygiene activities (including, but not limited to: helping patients to shower, bathe, change clothing, shave, oral care, etc.)
  • Observe patients and reports observations and concerns to the patients care team
  • Complete patient records accurately and in a timely manner
  • Assist in maintaining a clean, healthful environment for the patient.
  • Attend and participate in interdisciplinary meetings, in-services, and staff meetings as requested
  • Be willing and able to lift patients for routine hygiene as needed
  • Act quickly and responsibly in cases of emergency
Qualifications:
  • Enjoy working in a one-on-one setting, eager to improve and continuously learn new skills
  • Ability to perform all essential job functions with or without accommodations
  • Valid driver's license, vehicle insurance and access to a dependable vehicle every single day
  • Validated ability to act in a compassionate and supportive manner
  • Available to work weekends
  • Prior experience in a healthcare, hospitality, or in-home care environment (minimum of 1 year)
  • Willingness to enforce health and safety standards
  • Supportive and compassionate
  • Current CPR Certification
  • Must be a State Tested Nursing Assistant (STNA)
  • High School Diploma
  • Take pride in providing high quality care
  • Hoyer Lift Experience (specific to client)

  • This is a part-time not benefit eligible position (PRN) - possibility to advance to a full-time position if the future need arises.
