The Care Management Assistant will provide support to the Vandalia Health Network Population Health team. This position will collaborate through direct patient outreach, communication with network organizations, and post-acute providers to coordinate care. The Care Management Assistant will document all care management activities in the approved care management system. Minimum Qualifications: High school diploma or GED. Post high school certificate as a Medical Assistant, LPN, or Nursing Assistant. One year of experience in a patient care setting as a Medical Assistant, LPN, or Nursing Assistant required.
