Job DetailsJob LocationMessiah Lifeways - Mechanicsburg, PAPosition TypeFull TimeEducation Level4 Year DegreeJob ShiftDayJob CategoryHealth CareDescriptionJob Summary:The Residential Living Social Service Coordinator is responsible for promoting resident's well-being and satisfaction by supporting resident centered care, developing and interpreting standards for Residential Living, supporting quality of life and providing case management support to residents as needed by utilizing levels of care and services provided within Messiah Lifeways as well as community resources when appropriate.Job Duties/Responsibilities:
  • Participate in new resident orientation,, assist with transfers to other levels of care, and discharges within Residential Living, and provide support to residents and their family members/responsible parties as physical, social and emotional concerns or needs arise
  • Responsible for the initiation and maintenance of any Residential Living resident's Aging in Place Addendum through collaboration with resident, family, and Residential Living team while utilizing social work skills that promote person directed choices
  • Provide leadership and guidance to other team members modeling quality of life enhancement practices and services that support the values of person-centered, elder-directed care
  • Conduct and attend meetings in conjunction with job responsibilities
  • Participate as a team member to address level of care changes by attending Admission Committee meetings, therapy meetings, discharge planning meetings, and other identified meetings that assist with this process
  • Participate in planning Residential Living programs and assist with policy and procedure revisions by observing current policies and informing Residential Living Administrator of gaps in programs
  • Work collaboratively with the other levels of care to ensure a smooth transition for each resident while maintaining a positive working relationship with fellow team members
  • Complete Electronic Health Record documentation timely and as appropriate
  • Periodically meet with new residents to assess customer satisfaction and that their needs are met
  • Provide information and referral services to residents as a part of case management
  • Advocate for residents' rights
  • Keep up to date with legislation, trends, ideas, resources, and facilities related to Residential Living in other CCRC and independent settings
  • Assist with leading a monthly caregiver support group for residents who are primary caregivers for their spouse/loved one
  • Follow emergency procedures and respond rapidly and appropriately to emergencies ensuring the safety of the residents and team members
  • Follow proper procedures and promote safe work practices in order to maintain a safe working environment
  • Confidentiality regarding resident information must be maintained in strict confidence, as well as other information pertaining to the business of Messiah Lifeways
  • Additional duties and tasks as assigned by the Residential Living Administrator
  • Provides on-the-floor time and coverage, as needed
QualificationsRequired Skills/Abilities:
  • Proficient office, organizational, and computer skills
  • Knowledge of social casework principles and methods
  • Ability to plan and organize programs
  • Demonstrate leadership and effective communication skills
  • Demonstrate ability to set and achieve goals
  • Must have patience, tact, cheerful disposition and enthusiasm
  • Ability to work in a team environment
  • Ability to accept direction and supervision
Education/Experience:
  • Bachelor's degree from an accredited school of Social Work preferred
  • Two (2) years' experience in a long term care setting
  • Maintains a current, valid, Pennsylvania driver's license
