McHarrie Life is seeking a skilled, energetic Case Manager to join our team in our full timeday shift opportunity. Responsible for the discharge planning process in our facility's short term rehabilitation unit. This position includes providing support to residents and family members through their admission, stay and discharge from facility as well as working with interdisciplinary team members on daily admissions and discharges.

Preferred qualities for successful Case Worker candidates include:

• Bachelor's degree in social work, human services or related field.
• Two years' experience in a health care setting.
• Experience facilitating all aspects for a safe discharge into the community or other appropriate services including arranging for skilled home care, therapy and adaptive equipment as needed.
• Experience with care planning and MDS assessments.
