CCFHS is looking for a Team Coordinator to join our Medical Practice working in our Colebrook location!

This position is Monday through Thursday, 10 hours per day and 40 hours per week. Full-time Benefits Available.

Generous Earned Time benefits accruing up to 4 weeks per year!

SUMMARY

This position is expected to work closely with CCFHS's Office Manager and is supervised by the Operations Manager Colebrook.

Communication

* Responsible for screening and directing telephone calls to teams; transferring calls to triage, clinical staff and/or other departments.

Team Coordinator

* Responsible for greeting patients; screening and directing telephone calls, inquiries, and enrolling patients for services; updating patient demographics, insurance information, consent for services, personal contacts, HIPAA information, etc.; posting payments, co-pays and other related tasks as assigned; and working with clinical teams to address patient services and satisfaction.

Performs additional functions unique to the Colebrook office such as

* Maintain an inventory of office and clinical supplies and place orders when stock is low, ensuring the Colebrook office has adequate supplies for staff and providers to do their jobs and meet patient needs.

* Coordinate process when someone requests to become a new patient, ensure all steps are followed.

* Track patients who request a sliding fee application. Complete daily no show/same day cancellation tracking and report this information to the Operations Manager weekly including any trends identified.

Qualifications

High school diploma or GED equivalent.

Must be organized and efficient, with solid typing skills.

Must demonstrate ability to work well with the public.

Strong Customer Service Skills.
