Job Summary:
The Center for International Health Advanced Practice Provider will serve as a Full Time APP to provide care to patients within the Center for International Health at SUNY Upstate NAPPI Downtown Campus.
The APP would be responsible for providing primary care to resettled refugees in the community.
Responsibilities include but not limited to:
+ Providing expert level clinical care & coordination of care.
+ Provide full range of services that will improve quality of patient care assessing & evaluating patients.
+ Ordering appropriate laboratory testing & diagnostic studies.
+ Arranging for referrals, testing, consultation & therapeutic services,
+ Maintaining outstanding relationships with families, referring physicians & all members of the health care team.
As an APP you are a member of the multidisciplinary healthcare team which demonstrates professionalism and leadership within the institution, exemplifying the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.
Minimum Qualifications:
Master's Degree as a Nurse Practitioner or Physician Assistant.
NYS Licensed/Registered NP or PA.
Current National Certification consistent to the clinical practice.
NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
CPR-BLS Certification required at the time of appointment.
Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
1-2 years primary, ambulatory experience.
