The Pavilion at Robinson Terrace has the following openingRN Case Manager-Assisted Living Facility, $1,500 Sign-On; Travel AllowanceJob descriptionJob Summary:We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Case Manager, you will be responsible for coordinating and providing comprehensive patient care, ensuring the highest quality of care and patient satisfaction.Duties:- Conduct patient assessments and medical history review- Collaborate with physicians and healthcare professionals to develop individualized care plans- Monitor and evaluate resident progress, adjusting care plans as needed- Provide education and support to residents and their families regarding their healthcare needs- Coordinate admission and discharge planning and ensure smooth transitions of care- Maintain accurate and up-to-date medical records, adhering to HIPAA regulations- Collaborate with interdisciplinary teams to ensure effective communication and coordination of care- Utilize case management principles to optimize patient outcomes-initial and periodic evaluation, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs;-orientating a new resident and family to the daily routine;- assisting each resident to adjust to life in the facility;- Identify persons in need of services and assist external service providers in establishing a relationship with these residents;-Work with these service providers in executing a plan for service for individual residents; and-Assist in arranging for transportation as necessary to ensure that residents are able to attend required services provided in an external location.-Each resident shall be provided such case management services as are necessary to support the resident in maintaining independence of function and personal choice.-The operator shall establish a system of recordkeeping which documents the case management needs of each resident and records case management activities undertaken to meet those needs.Experience:- Minimum of 1 year of experience as a Registered Nurse-Strong knowledge of resident t assessment techniques and clinical skills- Familiarity with admission and discharge planning processes and resources- Proficient in electronic medical record systemsSkills:- Excellent communication and interpersonal skills- Strong critical thinking and problem-solving abilities- Ability to work collaboratively as part of a multidisciplinary team- Detail-oriented with excellent organizational skills- Ability to prioritize tasks and manage time effectivelyWe offer competitive pay rates, comprehensive benefits package, including medical, dental, vision, retirement plan, paid time off, and professional development opportunities.If you are a compassionate Registered Nurse with a passion for providing exceptional patient care and possess the necessary skills for this role, we encourage you to apply.Job Types: Full-time, Part-timePay: $75,000.00 - $90,000.00 per yearExpected hours: 40 per weekBenefits:401(k)401(k) matchingDental insuranceHealth insuranceLife insurancePaid time offVision insuranceSchedule:8 hour shiftDay shiftMonday to FridayOn callWeekends as neededAbility to Relocate:Stamford, NY 12167: Relocate before starting work (Required)Work Location: In personrecblid e8o7221ds8uo5qlt3drtoua8gy0mt0
