*CARE COORDINATOR*Interborough Developmental and Consultation Center is hiring Care Coordinator/Care manager for its expanding Adult Health Home Plus population.Adult Health Home Case Management Program which offers care coordination and resources for adults with two or more chronic conditions throughout New York City.The goal of the Health Home program is to make sure its members get the care and services needed. Under the general supervision of the Program Director/designee, functions as part of an interdisciplinary team assisting assigned individuals in developing and maintaining viable living, working and social situations in the community by obtaining needed medical, social, psychosocial, educational, financial, vocational and other services.*SALARY RANGE:* $24-$26 hourly. Based on experience and licensure.*QUALIFICATIONS:** BA/BS Degree Required with three (3) years of experience, or two (2) years of experience as a Health Home care manager service in the SMI or SED population.* A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with two (2) years of experience* Strong written and verbal skills* Excellent computer skills are necessary, demonstrated ability to use word-processing, or data base programs.* Knowledge of community resources, and entitlements/benefits.* Good organizational and interpersonal skills.* *Bilingual Spanish/ Russian encourage to apply**This position requires the candidate to be on the field about 75% of the time.**RESPONSIBILITIES** Conducting home visits, field visits, phone and mail outreach to individuals referred for enrollment.* Reviews client intake assessment and uses results to coordinate the completion of the care plan, self-management goals and strategies; documents them in EMR* Travels to/visits consumers' residences or apartments to provide counsel and assistance to help the consumers, within their capabilities and interests, in maintaining the greatest degree of independent living.
