Job Description:The Registered Nurse position is responsible for ensuring clinical planning, coordination, assisting with COVID-19 testing and vaccination processes, and patient triage. The Registered Nurse is a critical care team member and works with nursing staff to plan, deliver and evaluate nursing care based on the needs and concerns of patients and administration. The Registered Nurse serves as a leader and maintains quality of care through an on-going process of assessment, planning, and implementation. Essential Duties and Responsibilities The following are essential responsibilities for the role:
- Review patient charts for missing labs, reports, and/or open orders
- Triage urgent same day or double book appointment requests
- Coordinate and participate in daily huddles
- Assist with inventory management.
- Prepare equipment and aid physician during treatment, examination, and testing of patients
Verify chief complaint and take vital signs as requestedReview the EHR active medication list with patientsConduct and document point-of-care tests based on protocolPerform injections as directed by provider- Provide general nursing care to patients:
Administer prescribed medications and treatments in accordance with nursing standardsPlan, articulate, and coordinate care and interventions- Maintain consideration for specific age related characteristics, needs related physical development, communication, and socialization and safety, in collaboration with other member of the care team
- Assist with the COVID testing and vaccination processes
- Facilitate clinical messages and chronic condition management education
- Provide abnormal test results to patients and provide recommendations on cases PCP determines
- Add recommendations to individualized care plan(s) and review After Visit Summary (AVS) with patient
- Document progress notes in electronic health record within 24 hours
- Manages patient flow through primary care clinic
- Perform CLIA waived tests as needed
- Maintain all environment of care quality logs
- Participate in scheduled clinic rounds to maintain the environment of care
- Conducts medication management including performing medication reconciliation, reinforcing medication knowledge, and understanding medication regimen; refers to pharmacist as required
- Enforces adequate Infection Control standards, with guidance from the Medical Director.
Education and Certification
- Must be a Registered Nurse licensed by the State of New York.
- A Bachelor's degree from an accredited college or university or an Associate Degree in Nursing with 2 years' experience.
