Job Description :
As a Community Liaison, you will assess, plan, implement, monitor, and evaluate services that are required to meet a client's needs. You will provide supportive counseling services including day to day management and problem-solving assistance as well as behavior management interventions to clientsaccording to their individualized treatment plan.Benefits for Full-Time Employees:

Qualifications:
  • Bachelor's Degree in Social Work, Psychology, or related behavioral health services field
  • Minimum of one (1) year experience or equivalent school/community service experience with at-risk youth/families, knowledge of and experience in trauma- focused treatment and child development
  • Must maintain an appropriate amount of continuing education per policy
  • Ability to travel, valid driver's license and insurance statement
Responsibilities:
  • Provide supportive counseling, targeted case management, behavior management (BMI) and crisis intervention as prescribed in the treatment plans.
  • Coordinate, in collaboration with the Clinical Supervisor and other treatment team members
  • Completes the CANS and other screeners or assessments as indicated by the treatment team
  • Assist with the development, implementation and review of every treatment plan and any other service plans in accordance with state and agency requirements
  • Meet weekly services hours as determined by supervisor.
  • Assist in the implementation of new service delivery systems/programs developed by DHHR or other corresponding state agencies.
  • Engage and communicate effectively with youth and their families/legal guardians, and all team members.
  • Assess and respond effectively to crisis situations.
  • Collaborate with team members to access the needed supports and services for each youth and family served by working with youth and family-serving agencies and organizations in the community.
  • Advocate for youth and their families by attending appropriate meetings, such as MDT's and IEP's
  • Adhere to confidentiality, HIPAA, mandated reporter, and risk management policies and procedures including but not limited to completing Incident Reports.
  • Utilize technology provided in order to perform job efficiently and effectively
  • Complete service activity logs (SALs) and client documentation in accordance with agency standards.
  • Must possess current certifications for all core trainings (ex. CPR, CPI, and First Aid)
