Compass Healthcare Consulting & Placement is conducting a search for an experienced RN Director of Patient Services for a Home Health Hospice Program that provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospice receives patients from referring institutions throughout New York City and the entire tri- state area. Qualified candidates will be a NYS RN with extensive experience within administration and clinical management and will have prior Hospice, Palliative Care and/or Certified Home Health Agency (CHHA) experience with Interdisciplinary Teams, Medicare CMS, preferred.

RN Director of Patient Services

Assists in the overall administration and management of the daily clinical and operational functions of the Hospice Program. To coordinate, direct, supervise and evaluate the coordination of patient care services consistent with the philosophy, policies and standards of the Agency. The Director of Patient Services is responsible for the clinical operations of the Hospice and Home Care. Provides leadership and coordinates the clinical activities of the interdisciplinary team. Responsible for the provision of skilled IDT assessments and the implementation of coordinated clinical care by all disciplines of the Hospice and Home Care teams.

Responsibilities:

  • Elicits recommendations regarding continuous improvement of patient safety from staff.
  • Encourages reporting of occurrences by staff without fear of reprisal.
  • Participates in the development of recommendations for improvement in patient safety based on staff recommendations.
  • Participates in the implementation and monitoring of patient safety programs.
  • Coordinates all activities of the interdisciplinary team.
  • Provides day to day support to Patient Care Coordinators in management of individual interdisciplinary teams.
  • Provides supervision of Patient Care Coordinator performance.
  • Assists in the identification of areas for individual and agency improvement and in the development, implementation, and evaluation of plan(s) for staff development and improvement.
  • Participates in orientation and evaluation of new hospice staff and existing hospice staff who have changed roles.
  • Relates staffing needs to the Hospice Administrator and Human Resources.
  • Interviews potential staff members, makes recommendations for hiring decisions.
  • Makes recommendations for personnel changes such as hires, transfers, promotions, and terminations.
  • Coordinates all activities of the interdisciplinary team.
  • Directs the development and review of coordinated plans of care.
  • Provides consultation to staff in assessment, planning, implementation and evaluation of patients and families.
  • Monitors daily staffing practices.
  • Provides direct oversight of Fee-For-Service staff scheduling and assignment to a particular team based on staffing needs.
  • Direct daily oversight of Intake Staff to ensure that potential patients are appropriate for hospice benefit or for appropriate care throughout various programs within the agency.
  • Assists in the process of securing adequate clinical information for admission consideration.
  • Interacts with referrers, including family members, to provide explanation of hospice services and to ensure safe and appropriate transition to hospice.
  • Acts as a liaison between the Intake office and Patient Care Coordinator and their teams.
  • Participates in agency planning activities. Acts as project manager as assigned for specific project improvement initiatives.
  • Serves as Administrator on Call as scheduled.
  • Actively participates in Quality Assurance/Performance Improvement and Utilization Review activities.
  • Conducts medical records audits, participates in needs analyses, oversees new/revised practices as assigned.
  • Assists Hospice department managers and PCCs in decisions related to patient care and personnel decision making.
  • Interacts with these staff members in a supportive manner throughout the organization.
  • Counsels staff as needed and maintain documentation of the same.
  • Represents company in professional activities.
  • Monitors on-call records to assist in the identification of priority needs and plan for daily agency activities to meet these priority needs.
  • Initiates measures to meet the 24/7 needs of patients and families.
  • Ensures ongoing care plan and visit adjustments to meet the needs of patients during hospice episodes of care.
  • Responds to patient/family concerns and as necessary, institutes actions to mitigate/resolve issues to promote optimal care.
  • Works within legal, ethical practice standards to his/her position as well as Company Code of Conduct.
  • Complies with appropriate policies and procedures and brings potential compliance matters to the supervisor or compliance officer.
  • Participates in ongoing evaluation and development of additional programs.
  • Rotates as Agency Administrator on call.
  • Direct oversight of all On Call staff and On Call Availability staff.
  • Coordinates scheduling of all On Call Staff and Administrator On Call Staff.
  • Ongoing review of pharmacy invoices to ensure accuracy and track all invoices for completion and processing by Accounts Payable.

Qualifications:

  • Current N.Y.S. R.N. License
  • Baccalaureate Degree in Nursing from an accredited school is required, Master's Degree in health-related field preferred.
  • Minimum of six (6) years of professional nursing service, which should include at least two (2) years in Community setting. Minimum of two (2) years supervisory experience in community setting.
  • Current Driver's License preferred and insurance
  • Ability to communicate effectively and sensitively with other staff members, referral sources, patients and their families and physicians.
