Position Responsibilities:

* Monitors patient activities during treatment time under the direct supervision of the therapist.

* Transfers and/or assists therapist/nursing personnel in transferring patients utilizing appropriate body mechanics, as observed by therapist.

* Transports patients, on a timely basis, according to schedule, and communicate with the therapist if there is a problem getting the patient to his/her therapy session on time. Check patient charts for presence of therapy orders before transporting patient.

* Prepares or removes hot/cold packs from the patients and checks patient's skin as directed by the therapist and/or modality protocol.

* Assures that patients are properly dressed or draped for transfer and during treatment.

* Inspects equipment prior to patient use and notify the therapist of any problems, such as maintenance or sanitary.

* Cleans equipment with appropriate hospital approved disinfectant, and maintain a log as specified by P & P and supervisor.

* Observes patient one-on-one to ensure comfort and safety.

* Consistently monitors patient's personal needs, recognizes and reports observations to the primary/charge nurse as required.

* Communicates with patient and/or family to understand patient's needs and wants.

* Independently identifies potentially unsafe environments for the patient; proactively takes initiative to prevent or resolve through appropriate channels or chain of command.

* Assists with basic patient needs that do not require further formal training (i.e. feeding the patient).

* Remains awake and alert through the complete assignment.

* Successfully completes competencies as outlined by the departments operations as relates to age specific population and JC requirements.

* Acts as a liaison between Rehab-aides/Transporters and other supervisors to assist in resolving transport issues as observed by supervisor.

* Coordinates with supervisor, Rehab-aides/transporters time off to ensure appropriate coverage for weekdays and weekends.

* Actively coordinates duties among the Rehab-aides/Transporters to ensure maximum efficiency of patient care in the absence of the supervisor as observed by Manager.

* Attends and participates in in-service training, orientation and educational programs.

* Assures that assigned inventories and logs are completed on a routine basis as specified by the area supervisor. Also maintains inventories (order supplies, forms) in a timely basis.

* Participates in Performance Improvement projects.

* Is willing to perform other related duties.

Qualifications:

* CPR certification required.

* HS graduate or equivalent. Effective oral and written communication skills. Successful completion of the departmental competencies.

* Ability to relate to patients in a kind and cooperative manner.
