Job Details- Primary Care Clinic at Tsehootsooi Medical Center.
- Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic.
- Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays.
- Collaborate with support nursing staff and with providers for patient care.
Job Requirements- Three (3) year Case Management Experience.
Schedule Information- Weekdays Monday through Fridays from 8a to 5p.
- One (1) hour lunch.
- No weekends or holidays.
Additional Details- Travelers are occasionally expected to float to other departments in which they are qualified to work.
- This could include taking a patient load if experienced/competent in that setting or could be task-oriented such as answering call lights, phones, etc.

- Charge Rates: travelers may be asked to work charge, no additional charge pay.
- Orientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following Monday.
