If you want to make a difference in the lives of others, than Spring Creek is where you should be!*Spring Creek is an equal opportunity employer.Job duties for our CNA's include but not limited to the following:- Provides, assists or supervises patients with self-care as outlines in plan of care allowing patients time to do for themselves as able, including: grooming, dressing, bathing, nail care of non-diabetic patients and applying male external catheters, emptying catheters/leg bags, all of which promote self-worth.-Answers call lights promptly to assure patient's needs are met.-Performs post mortem care as needed to prepare the patient for funeral home employees.-Obtains patient's weights and leg measurements (on nursing units) as ordered, according to the patient's plan of care.-Provides perineal care for incontinence in a timely and supportive manner to enhance hygiene and good infection control practices.-Assists with feeding infirm or ill patients or provides adaptive equipment for self-feeding as outlined in plan of care for each meal and documents in Point of Care or food intake monitor.-Assists appropriate patients at designated areas in the dining room or patient rooms to provide verbal cues regarding feeding/swallowing strategies and monitors modified diet/liquid levels.-Report patient's response to treatment activities to nurses within established time frames.-Uses gait belts to assist patients in transfers, gait training and functional ambulation as needed utilizing transfer status on patient medical record and plan of care to promote safety and carryover from therapies.-Establishes communication to meet patient needs by creating a meaningful relationship with the patient and family by using verbal and non-verbal communication tools.- Maintains constructive communication and relationship with co-workers to enhance a friendly, supportive atmosphere by initiating discussions and participating in unit/department meetings and care conferences as assigned.-Suggests methods to improve patient care by discussing with patient care team or nurse on duty.-Maintains confidentiality of patient information at all times and is aware of any audience that may surround any confidential conversation to ensure proper HIPAA compliance.-Maintains supplies as needed by the patient or assigned area.-Assists with transportation of the patient to activities, appointments or to therapies as needed.-Assists with extra duties as assigned (kitchen cleanliness, shower cleanliness, linen room cleanliness, trash emptied as appropriate, lifts stored in proper designated areas, linen carts stored in appropriate areas, isolation carts stocked and located in appropriate areas, etc.) as designated per unit.-Operates and maintains all equipment as per manufacturer guidelines and facility policy.-Works closely with members of the patient care team, families and other facility employees.-Other duties as assigned.
