DescriptionYour Experience Matters – New Higher Salaries Based on ExperienceSalary Range (Annualized): $70,720- $76,960Maintains the delivery of health care services for individuals with developmental disabilities who are living in community based homes. Provides/participates in agency training.ESSENTIAL FUNCTIONS/RESPONSIBILITIES;• Monitors the delivery of preventive and routine health care for individuals with developmental disabilities.• Provides preventative, routine, and emergency healthcare for individuals with developmental disabilities.• Adheres to and ensures facility compliance with all Physicians, NP, PA etc. orders. • Accompanies individuals on appointments, if assigned. • Completes medication administration in the facility.• Within each month, completes a monthly breast exam on female individuals as scheduled. • Provides health service training for staff. • Provides agency training as requires.• Maintains compliance with all applicable state/federal regulations and agency policy and procedure. • Maintains communication with RN Clinical Coordinator, Senior Residential Supervisor, Residential Director and other staff as necessary.• Assist in completion of daily logs, pertinent forms and documentation.• Assist individuals as needed in all activities and areas of daily living.• Completes all areas assigned through shift responsibility sheets to ensure health and safety of individuals. • Performs housekeeping duties and laundry as assigned. • Participates in pre-shift meetings, semiannual and annual reviews, and house staff meetings as required. • Attends all agency mandated in service training sessions in appropriate time frames. • Attends all site specific trainings as mandated by the RN Clinical Coordinator and/or the Director of Nursing for the Agency.• Required to fulfill pre-scheduled on-call commitment as determined by RN Clinical Coordinator and/or the Nursing Director for the Agency. • Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R). • Complies with all agency policies and procedures.• Other duties as assigned.MINIMUM QUALIFICATIONS;• Current NYS license to practice as a Registered Nurse.• NYS Driver's License that meets agency policy.• CPR certification.• Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.• Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention Core Training certification as per regulatory guidelines.
