Title: Registered Nurse – Support and Connection Center ( SCC)
Schedule: 12h rs Day Shifts. Schedule flexibility , including weekends.
Salary: $95,000
Position Overview:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our team at The Support and Connection Center . The Support and Connection Center offers medical and social services to individuals affected by homelessness, mental health challenges, and substance use. The RN reports directly to the Nurse Manager of the Support and Connection Center; i ndirectly reporting to the Director of Nursing Operations , and the Program Director of the Support and Connection Center ,
Essential Duties & Responsibilities:
Direct Client Care:
Conduct comprehensive health assessments for clients with substance use disorders and co-occurring mental health conditions, considering physical, mental, and social factors.
Administer medications , a ssure safe storage of medications , m aintain accurate documentation of medications , assists clients with self-administered medications , provide medication teaching .
Education and Counseling:
Educate clients on harm reduction practices, overdose prevention, and mental health self-care strategies.
Collaboration and Coordination:
Work collaboratively with multidisciplinary teams including psychiatrists, social workers, occupational therapists, peer advocates, and rehabilitation aides.
Coordinate referrals to community resources such as addiction treatment centers, mental health services, medical services, and housing support.
Advocate for clients within the healthcare system to ensure integrated and comprehensive care.
Documentation and Reporting:
Maintain documentation in compliance with agency policies and procedures
Participate in team meetings to discuss integrated client care and treatment outcomes.
Qualifications:
NYS RN License
Working with unhoused individuals preferred.
Community Health s ettings preferred.
Compassionate, non-judgmental approach to healthcare delivery.
Experience or strong interest in working with individuals affected by substance use disorders and mental health conditions.
Knowledge of harm reduction principles and practices .
Preferred skills:
Communication: Must have excellent oral , listening , and writ ing skills.
Interpersonal skills: Must have the ability to interface with clients as well as all levels of multidisciplinary staff.
Computer skills: Must have knowledge of Microsoft Office .
