Acentra seeks a Clinical Evaluator to join our growing team.

Job Summary:

As a Clinical Evaluator (PASRR) - Preadmission Screening and Resident Review, you will conduct face-to-face assessments to facilitate appropriate placement and next level of care in Port Angeles, Washington. Your role involves providing essential evaluations to enhance patient well-being and improve their quality of life. These assessments, primarily conducted in nursing homes, hospitals, or community-based settings, play a crucial role in determining the level and scope of care required for each patient.

As a trusted expert, you will evaluate, determine, and recommend the next level of care to improve overall quality of life. This is a (Part-Time) Hybrid role, combining work from your home office with travel to evaluate patients in your local area, including the Port Angeles, Clallam County or Jefferson County. Travel expenses, all mileage and parking will be fully reimbursed.

Job Responsibilities:

+ Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations

+ Assess and analyze psychological, emotional, behavioral and physical functioning.

+ Develop informed level of care recommendations based on thorough clinical evaluations.

+ Conduct assessments in various environments, including nursing homes, hospitals, and community settings, to understand and address the diverse needs of those we serve.

+ Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.

+ Ensure compliance with contractual and regulatory standards while developing level of care recommendations.

+ Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.

+ Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.

+ Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.

+ Monitor personal metrics to ensure compliance with turnaround time requirements.

+ Stay updated on current regulations, and research related to long-term care, mental illness, medical conditions, and geriatrics.

+ Efficiently prioritize and execute tasks with attention to detail.

The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary.

Required Qualifications/Experience:

+ Minimum 2 to 3 years of experience assessing individuals with mental or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive issues, and dementia.

+ Active unrestricted licensure in Washington in a healthcare or related field, such as a Licensed Psychologist, Psychiatric Nurse, Social Worker, Mental Health Counselor, Marriage and Family Therapist, or Social Worker Independent Clinical License.

+ Graduation from an accredited college with a relevant degree program.

+ Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning.

+ Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per Federal Regulation 42 CFR, Chapter 483.

+ Knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual in use in Washington.

+ Experience in determining a clinical diagnosis accounting for psychological and behavioral issues.

+ Knowledge of standards of practice related to the determination of Level of Care and standards of practice related to mental health, geriatrics, and other disorders commonly necessitating long-term care placement.

+ Experience with public mental health and medical programs, as well as public sector services and supports..

+ Ability to interact with healthcare professionals, patients, their families, and other support systems.

+ Experience working with the geriatric population.

+ Excellent communication skills for conveying information to individuals and groups through spoken, written, and electronic media.

+ Attention to detail, with the ability to prioritize and execute tasks efficiently and in a timely manner.

+ Proficient with Microsoft Office applications, including Excel, Word, PowerPoint, Teams, Outlook, and other web-based software

Compensation

The compensation for this role is $40.00 to $44.00 per hour.

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
