Introduction GRADE 14 LOCATION OF POSITION MDH - Kent County Health Department 300 Scheeler Road Chestertown, MD 21620 Main Purpose of Job The purpose of the Coordinator of Special Programs, Health Services II is to provide resource coordination services to eligible individuals with Developmental Disabilities who are residents of Kent County. This position determines eligibility for DDA services, provides referrals for services, accesses day/residential/support services, facilitates the annual individual plan meetings, assists the served individuals and their families to develop an annual individual plan, monitors individuals on the waiting list and assists in transition planning. These efforts work to maintain the individuals in the community and to promote individual choice and their overall quality of life. MINIMUM QUALIFICATIONS Education: Graduation from an accredited high school or possession of a high school equivalency certificate. Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources. Notes: 1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. 2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education. DESIRED OR PREFERRED QUALIFICATIONS The desired candidate should possess one of the following: A bachelor's degree from an accredited education program in a human services field An associate's degree with two years experience in a human services field Seven years experience in a human services field SELECTION PROCESS Applicants who meet the minimum (and selective) qualifications will be included in further evaluation.
