***New flexible pay ranges****Keota Health Care is seeking an MDS Assessment Coordinator for our Skilled Nursing Facility in Keota, IA!The primary purpose of the job position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of the state and the policies and goals of this facility.This is a full-time position, part time as MDS and 1 day per week as a nurse on the floorSign On Bonus: Earn up to $10k based on position/shift acceptedBuddy Bonus: $1000 for every FT or PT nursing staff member you bring with youResponsibilitiesRoutinely assess residents according to pre-set schedules, or on an as needed basis for condition changes, hospital return, etc.Responsible for accurate observation, assessment, and communication of condition changes to appropriate personnelAssures completion of assessments and CAA's by other departments such as Social Services, Activities, and Dietary, and obtains corresponding signaturesComplete other sections of MDS, quarterlies, CAA's and care plans, etc., in the absence of other personnelInitiate care plans based on resident needs identified in the Resident Assessment Protocols and update the care plans according to guidelinesPrepares and distributes MDS schedule to interdisciplinary teamTransmit MDS to the state weekly (or as required) and maintain accurate records, including a transmittal logPlan, organized, develop, and direct the overall operation of the Nursing Service DepartmentParticipate in interviewing and screening residents for admission to the facilityReview nurses' notes to ensure proper documentation is maintained relating to resident's treatment, medications, and conditionsMake daily rounds for observation of the care of residentsTalking with physicians and visiting selected residentsAssist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the MDS, including the implementation of RAP's and triggersQualifications:Registered Nurse RN of this stateOver a year's experience in Long-Term Care as an MDS assessment coordinator.Pay: $40.00 per hour.
