**Job Title:** Registered Nurse**Job Specialty:** Home Health**Job Duration:** 260 Days**Shift:** Day - 5x8 - 09:00 AM - 05:00 PM - 40 Hours**Guaranteed Hours:** N/A**Experience:** Two (2) years in a home setting within the last fifteen (15) years**License:** Current RN Licensure in the state**Certifications:** BLS required before start date**Must-Have:** Recent experience conducting comprehensive assessments of patients; ability to work with technology in the workplace; strong organizational, interpersonal, written, and verbal communication skills**Job Description:**- Review health status questions and make evaluations based on established criteria- Perform initial in-home pre-evaluation of patients, including assessment of health status, strengths, care needs, and preferences- Guide development of individualized long-term care service plans- Review consumer medical documentation or health referral forms, relevant to the case- Enter evaluation data into electronic form and transmit as required- Document concerns, conflicting information, or issues during the evaluation process- Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and treatment gaps- Discuss healthcare options, supports needed, service vendor options, and waiver options with consumers- Initiate communication for enrollment into a managed care health plan- Enhance communication and collaboration with interdisciplinary care team members to improve care coordination- Facilitate service delivery and collect quality review data- Record case notes and identify opportunities for health promotion and illness prevention- Maintain knowledge of community resources and requirements for the target population- Meet standards established for the position as outlined in the annual performance criteria and bonus template- Perform additional duties as assigned by managementPlease apply with your resume and include relevant experience and qualifications.
