Our mission is to nurture stronger familias to live out their dreams.El Futuro is currently hiring full-time, Bilingual Community Health Workers in Rural Chatham County and surrounding areas. Community Health Workers provide services to support community partnerships to improve access to behavioral health care for Spanish speaking, underserved populations, including the delivery of interventions created by Licensed Clinical Therapists. Delivery of interventions take place on-site at community-based partner organizations, primary care clinics, or other locations; and under the guidance of a Licensed Clinical Supervisor.This position works 37.5 hours per week, primarily in-person, at various locations throughout Chatham County and surrounding areas; must be able to travel to the Durham Clinic for meetings, and must be available for Friday morning meetings, 9:00-11:00am. The typical weekly schedule includes evenings and weekends (work on the weekend will begin approximately 3 months after hire).The pay rate is $22.00 per hour. El Futuro provides employer paid medical, dental, vision, life, and long-term disability; 403B matching; generous PTO and paid holidays; paid family medical leave; paid parental leave; and stipend and paid time off for professional development.This position starts on January 6, 2025.ESSENTIAL FUNCTIONSThe Community Health Worker performs a wide range of functions including, but not limited to the following:
  • Conduct screenings to identify participants' behavioral health needs and help with study participant recruitment and enrollment.
  • Maintain a consistent caseload of participants through the implementation of a brief, structured intervention focused on depression and anxiety, delivered weekly in-person or via phone.
  • Typical caseload is 12 to 16 sessions per week
  • Administer psychological symptom measures and track participants' medication compliance and side effects.
  • Facilitate connections between participants and community agencies and resources to help participants meet basic needs, such as food and employment.
  • Work collaboratively with other health care staff consistent with an integrated care model.
  • Track caseload status and progress using an online tracking system and assist with any reporting requirements.
  • Engage in community outreach activities, minimum one per month.
  • Provide a minimum of three Gatekeeping QPR (Question, Persuade, and Refer) training sessions per year.
General Responsibilities for El Futuro Staff:
  • Participate in meetings and organizational related activities as directed by the supervisor, which may include All Staff Meeting, Individual Supervision, participation on a committee, or other activities.
  • Play an active role in the continued development of El Futuro's work via participation in fundraising and other agency-wide activities.
  • Engage in learning opportunities through La Mesita , El Futuro's training platform and network of bilingual providers in North Carolina, or through other appropriate career development opportunities.
EDUCATION and/or EXPERIENCE
  • High school diploma or GED required.
  • Interest in mental health and in integrated health care.
  • Work or volunteer experience working as a Community Health Worker or similar experience as a natural helper.
  • Experience working in El Futuro's service area or familiarity with the service area and its resources
  • Required: Bilingual fluency in spoken and written Spanish and English
  • Experience working with the Latino community, preferred.

  • Excellent communication, interpersonal, and leadership skills.
  • Able to work autonomously as well as collaboratively in a dynamic, multicultural setting.
  • Skilled in managing multiple tasks and priorities with meticulous attention to detail and organizational proficiency
  • Demonstrated flexibility, creativity, and adaptability to cater to evolving client and organizational needs.
  • Proficiency in Microsoft Office, electronic health record systems, and project management systems (El Futuro uses IMS and monday.com).
  • Able to travel throughout El Futuro's service area.
