Under the direction of the Program Supervisor, the Community Health Worker will perform the following duties: Assess member needs in the areas of physical & mental health; SUD; oral health; palliative care; memory care; trauma- informed care; social supports; housing; vocational/employment; wellness; and referral and linkage to community-based services and supports. Utilize evidence-based practices, such as Motivational Interviewing, Stages of Change, Harm Reduction Techniques, and Trauma- Informed Care principles. Under the direction of the Program Supervisor and/or Clinical Consultant, the Community Health Worker (CHW) provides direct service, such as building care plans, completing assessments, as well as coordinates care management and functions as a part of a Care Team for the Enhanced Care Management (ECM), Community Supports (CS) or the Community Health Worker (CHW) services Programs. The CHW collaborates and communicates with members, works with caregivers/family support persons, other providers, and Community Support Teams in order to promote wellness, recovery, independence, resilience, and member empowerment, while ensuring access to appropriate services and maximizing member benefit. Conduct outreach and engagement activities to facilitate linkage to the ECM or CS program. Valid CA Class C Driver License in good standing and have personal, valid insurance and be able to be insured by the Center's insurance Carrier. Attend training as assigned (e.g., ACEs, MHC and CHW Certification). Given consent from the member advocate on behalf of members with health care professionals (e.g., PCP, etc.)QualificationsAny Combination of the following:1. Possession of an associate degree from an accredited college or university, with major collegecoursework in Public Administration, Business, Social Work, Psychology, Marriage andFamily Therapy, Emergency Management or a closely related field; OR2. Experience: Two (2) years of professional experience providing home health and/or social services management services to low-income populationsDESIRED QUALIFICATIONS: Skilled at administering programs that include formulating strategies, creating cross-departmental collaboration; building teams; and creating strong supervisory frameworks. Excellent interpersonal and communication (verbal and written) skills. Experience in health systems quality improvement program implementation and usingquantitative data to drive change. Basic knowledge of Medi-Cal programs and other public agency services administered througheligibility determination.
