Serve as a role model for the facility in quality improvement, operational management, and leadership. Works alongside the Facility Manager to achieve quality standards and operational performance at the facility, promotes teamwork and communication and fosters problem-solving skills. Maintains a positive work environment and stays focused on CDC's customers and their needs. Demonstrated knowledge of, performs, and evaluates correct practice of all CDC patient care policies and procedures and personnel policies and procedures. Is flexible and responds to the needs of the facility.Performance Expectations:Is knowledgeable of current industry standards and maintains knowledge base to provide leadershipDemonstrates a commitment to continuous quality improvement and provides education and support to improve outcomes throughout the organizationImplements plans to meet operational goals of facilityIncorporates CDC Cares into daily practiceCommunicates professionally with staff adhering to the CDC House Rules and PledgeMaintains confidentiality and professionalism in all interactionsCollaborates with Facility Manager in the evaluation process of all direct care staffActs as a resource for staffResponsible for the oversight, direction, and evaluation of all direct care staff that provide dialysis and nursing care in the facilityContributes to the hiring and terminating of direct care staffContributes to the facility's QAPI programFunctions as a staff RN as assignedQuality Management Responsibilities:Participates in and completes components of facility's QAPI programTrains staff on CDC quality program, standards, expectations and methods to improve resultsRoutinely reviews quality outcomes and works with staff to achieve CDC quality outcome goalsProvides leadership in implementing quality improvement initiatives in the facilitySupports the Facility Manager in assuring facility compliance with Medicare Conditions for CoverageEnsures program is compliant and ready for licensure and certification survey processImplements the Plans of Correction for licensure and/or certification surveysOversees medical record audits as directedReviews and provides input for revision of patient care policies as directedAssists in the leadership of facility quality teamsOperational Management Responsibilities:Assists with the implementation of patient care policies that are new or revisedCollaborates on solutions to address budget variances as neededImplements product and policy evaluation trials on site as directedMonitors adherence with CDC policy and procedureResponsible for holding monthly unit staff meetingsIs on-call for emergency issues at the facility as neededCollaborates with Facility Manager in the creation and maintenance of systems that facilitate smooth operation in the facilityAssists in the planning and implementation of plans to promote facility growthKeeps Facility Manager informed in a timely basis of unit activitiesCoordinates staffing resources with other Facility Managers, Director of Clinical Operations, and Central StaffingPatient Care Responsibilities:Functions in staffing according to schedule and patient load demands as neededEnsures all CIA/RIA/POC are completed according to CDC policyFacilitates intake of new patients utilizing CDC's centralized admission processDevelops and maintains professional relationships with referral sources and Medical DirectorSchedules and assures completion of new patient orientation per policyMonitors and assures patient education is completedImplements suggested dose guidelines and protocol dosing requirements for designated medicationsReviews monthly labs to ensure completion and identified issues are addressedEnters in RM Pro patient complaints and concerns and follows up on tasks assignedEnsures all policy and procedure is consistently followedSafety Responsibilities:Participates in the investigation of employee injuries to prevent and correct safety hazardsAssures all equipment and safety guidelines are adhered to in all operationsAssures new safety policies and procedures are fully implemented per guidelinesCompliance Responsibilities:Demonstrates knowledge of CDC Code of Ethics and Business Conduct and Compliance planImplements plans to assure facility compliance with CMS guidelinesEDUCATION, EXPERIENCE TRAINING AND LICENSES/REGISTRATIONS:Licensed as an RN in the state of OhioBSN preferredCNN or CDN preferredACN experience preferred6 months dialysis experience and 12 months experience in Clinical Nursing as a RN requiredDemonstrated commitment to quality careWorking knowledge of Quality Assurance & Performance Improvement (QAPI) processExperienced in Word/Excel/Outlook preferred#J-18808-Ljbffr
