- Provide a variety of office and field activities to coordinate, manage and monitor a community housing or placement program for individuals, performing direct client services and compiling all related documentation.
- Provide varying types of case management in various settings to individuals, families, and groups.
- Provide crisis intervention services as assigned.
- Studies and evaluates the housing needs of clients and matches those needs with the current available housing in the community.
- Researches and develops housing library or list of open/available properties to assist all candidates in obtaining permanent housing.
- Assist clients in securing financial move-in assistance such as security deposits, rental assistance, and eviction prevention systems, when available.
- Works to build relationships with all housing options throughout the community, such as property managers, private landlords and real estate companies.
- Possess a working knowledge of all homeless shelters in the area, community housing programs including low-income options, housing options for sex offenders or others with criminal backgrounds.
- Completes all agency documentation of services in a thorough, accurate and timely manner.
- Assists clients to adjust and adapt themselves to their environment, care, and treatment.
Qualifications CM I Must have or be willing to obtain Behavioral Health Case Management I certification. Valid driver's license required. Travel is required for this position. Mileage reimbursed monthly. CM II Bachelor's Degree in a Behavioral Science field required. Must have or be willing to obtain Behavioral Health Case Management II certification. Valid driver's license required. Travel is required for this position. Mileage reimbursed monthly.
